Evaluation of South West Healthy Communities Program

The South-West Healthy Communities Program (SWHCP) is funded by the Commonwealth Department of Health and Ageing. RHealth (formerly Southern Queensland Rural Division of General Practice) acts as the auspicing body for the program. SQHCP is based in Charleville and has a mission ‘to provide community wide access to holistic services specifically tailored to enhance the well-being of South Western Queenslanders, through a collaborative, integrated approach'.

CRRAH has been working with the programme since 2004 to develop and maintain tools required by SWHCP to collect and store their data. The developed forms and database allow SWHCP to easily enter and manage their large dataset and to report on their key performance indicators to the funding body.
