Nomination form completion guidelines

These guidelines have been compiled to assist employees in completing the nomination form for the Equity and Diversity Achievement Award and should be read in conjunction with the Employee Achievement Awards Policy and the Equity and Diversity Achievement Award Procedure.

Nomination form

Nominations must be made through the online Equity and Diversity Nomination Form. If you require any assistance with using this form please contact the HR Officer (HR Planning and Policy).

Applicants must indicate on the nomination form (by choosing an item for “Nomination type”) whether an individual or group of employees are nominating/being nominated for the award. If a group of employees is being nominated, the applicant must attach an additional page to the application with details of all group members. Please provide an overall name for the group on this additional attachment, for example “Capability Development and Training Team”.

Nomination details

Applicants must also choose whether the application is being made by self-nomination, by the nominee’s manager/supervisor or by a colleague/co-worker. If the application is being made by a manager/supervisor or a colleague/co-worker, the applicant must provide their details in the provided space.

Nominee contact details

The applicant must complete these details for the nominee (individual or the nominated contact person for group applications).

Title

If applicable, enter the name of the initiative, project or achievement for which the nomination is being made. For example “Development and Implementation of ClOVeR, a Creative Learning Online Via e Resources training site”. 

Diversity group

Applicants must indicate on the application which diversity group of the following key diversity groups, adapted from the Diversity Council Australia's (2014) key diversity areas, has benefited from the initiative/project/achievement:

  • women
  • people from culturally diverse backgrounds
  • people practicing different faiths
  • people with a disability
  • generational/mature aged
  • Aboriginal and Torres Strait Islanders
  • LGBTI (Lesbian, Gay, Bisexual, Transgender and Intersex)

Justification

Applicants must answer all of the justification questions, which will assist the evaluation panel in making their determination.

Referee support

Applicants must complete the contact details for two referees who may be called upon by the evaluation panel to provide more information about the application. These two (2) referees may be colleagues from amongst the University, students, other clients, or members of the wider external community. Referees would normally include one person from inside and another outside the nominee's work area (where possible).

Supervisor support

Where an employee has self-nominated, the nomination form must be supported by the employee's supervisor, who must view the applicant's nomination form, selection criteria statement and supporting information/evidence and may be called upon by the evaluation panel to provide more information about the application.

Selection criteria statement

Equity and Diversity Achievement Award selection is based on:

  • the extent to which an initiative/project/achievement has fostered the career enhancement or education of a diversity group at the University (the diversity group may be University students or employees)
  • the contribution the initiative/project/achievement has made to the development of a culture which values equity and diversity.

A statement addressing the selection criteria must accompany the nomination form (maximum of one (1) page), setting out the reasons for the nomination and clearly demonstrating the relevance and significance of the nomination by addressing the criteria. Applicants must demonstrate achievement of the selection criteria and provide specific examples and, where available, documentation of outcomes (which can be provided under the five (5) allowed pages of supporting information/evidence). As this statement is only one (1) page, it is important to be succinct in the selection criteria statement. Further information can be provided in the supporting information/evidence. 

Supporting information/evidence

Supporting information/evidence may accompany the nomination form. This information should not exceed five (5) pages. Examples of supporting information/evidence may include: work samples, evidence of outputs, and progress reports.

Referee support email

Referees must view the applicant's nomination form, selection criteria statement and supporting information/evidence and email the applicant indicating that they support the nomination. This email should accompany the nomination form. 

Supervisor support email

Where the applicant has self-nominated, the nominee’s supervisor must view the applicant's nomination form, selection criteria statement and supporting information/evidence and email the applicant indicating that they support the nomination. This email should accompany the nomination form.

Nominee acceptance email

Where the nomination form has been completed by someone other than the nominee, the nominator must gain the nomination acceptance of the nominee, who must view the nomination form, selection criteria statement and supporting information/evidence and email the nominator indicating that they accept the nomination. This email should accompany the nomination form. 

Documentation checklist

  • Fully completed nomination form
  • Statement addressing the selection criteria (maximum of one (1) page)
  • Supporting information/evidence (maximum of five (5) pages)
  • Email confirmation of referee support (Two referees)
  • Email confirmation of supervisor support (if applicable)
  • Email confirmation of nominee support (if applicable)
  • List of all group members and contact details (if applicable)

Call for applications and submission

The call for applications will be made annually from Human Resources. Documentation must be submitted on the Online Equity and Diveristy Nomination Form by the closing date. Late applications will not be accepted.

Process

Nominations will be reviewed by an evaluation panel comprising a minimum of three (3) members of the University, ie one professional employee, one academic employee, and the Chair of the Social Justice Strategy Board (or nominee) as Chair and will contain an appropriate gender mix. The evaluation panel will provide its recommendation/s to the Vice-Chancellor for approval. The University is not obliged to present each Award every year.

Awards

Awards are presented annually at the Staff Awards Ceremony. Each Achievement Award will consist of a Certificate and a Grant of Financial Support (available for professional development purposes). Conditions surrounding the expenditure of the grant are outlined in the Equity and Diversity Achievement Awards Procedure.

Commendation Awards may also be recommended in each category where there are nominees of sufficient quality. Commendation Awards consist of a Certificate of Commendation. No monetary grant is applicable.
