Complete an approval form for either animal ethics - research (DOC 241 KB) or animal ethics - teaching (DOC 213 KB) noting the following:
Submit an original (hard copy only) of the ethics approval form to:

A copy of each successful application requiring use of USQ Animal House facilities will be filed in the Animal House.
Once approved you will be required to provide an annual progress report (DOC 119KB) during your research and a completion report (DOC 119KB) at the end of your research to the Animal Research Ethics Committee. This is in compliance with the Australian Code of Practice for the Care and Use of Animals for Scientific Purposes which is mandatory under the Animal Care and Protection Act 2011 (QLD).
The questionnaire for monitoring research involving animals will be sent to you and will need to attach it to the report.
