To register for the Sustainability Alive Awards, students must complete the following registration and picture consent form:
Both forms and payment must be posted to the following address by Friday 7 August 2009:

Please do not fax entry forms as they must be accompanied by payment.
Cost
The entry fees for the Sustainability Alive Awards have been waived.
Individual entries - no entry fee
Group entries (2 or 3 students) - no entry fee
Entry Checklist
Entries are to be received by Friday 7 August 2009. If entries consist of parts (which could be separated), please ensure each section contains a label. All entries are to be labelled with:

Name;

School;
-
the division;
-
the category.
All entries must be accompanied by appropriate documentation as outlined in the specific category information. Students are advised to keep a copy of their written work.
