Process

1. Download the Registration form (PDF* 36 KB)
   
2. Complete the form, noting the number of tickets and additional tickets required.  
   
3. After completing the registration form, complete your payment details. If paying by bank cheque, please remember the amounts on the form are in Australian dollars and you will need to apply a conversion.  Also note that if paying by American Express a small surcharge will be added to the amount debited from your card.
   
4. Fax the form to us.

SMA has negotiated a special conference rate at the conference hotel should you wish to stay. Please see the Venue page for more information and booking details.

Social Functions

The one day forum registration fee will include:

• Morning tea and afternoon tea
• Full buffet lunch
• A cocktail reception

As part of the overall SMA08 conference, a Gala dinner will be held on Friday 18 July at the conference hotel. Delegates of the industry day are invited to attend the Gala dinner to allow continued networking opportunities. Tickets for this dinner can be added to the industry forum registration for an additional fee.  Simply select the number of tickets you require on the registration form and then calculate the final amount due.

Registration cancellations will not be accepted unless made in writing to the Conference Coordinator, either by mail, facsimile or email. Cancellations received four weeks prior to the conference will be refunded less 25%. No registration refunds will be made after 16 June 2008. As an alternative to cancellation, your registration may be transferred to another person in your organization and the Conference Coordinator must be advised of such transfer of registration not later than 48 hours prior to the commencement of the Conference.
