Report a Fault

- To report a fault or problem with any ICT supported products or services.

Change to Existing Product or Service

 - Change to Service or Product form is used to request fixes, enhancements, or any changes required to ICT approved products and services listed in the Products and Services Catalogue.

 

Request for Service

New Product or Service Request

 - To request for ICT to Host and/or support a new product or service. Product could be single application at the desktop or a network or web hosted solution. A service could be a number of products and requiring ICT support. these requests will be forward to the Change Advisory Board to review for acceptance.
