New Product or Service Request
The following information must be supplied to allow ICT to determine the resources required and perform an impact assessment prior to approval being granted to implement the service. (Note that a Service is defined as a new system, enhancement to an existing system, software application, hardware specification, or associated ICT service implementation.).
