Past Graduate Response form
Use this form if you didn't complete the online Graduate Response when you finished your academic program, and now need to claim your print testamur.
Replacement Testamur Request form
Use this form to request a replacement print testamur, or if you completed your academic program between (and including) Semester 2, 1991 to Semester 1, 2017 to request a digital testamur. If you completed your academic program from Semester 2, 2017, you have already been issued with a digital testamur which is hosted in the My eQuals portal.
Change of Name & Personal Details form
Use this form if you are a past graduate and unable to make an online Name Change Request via your Student Centre.
Authentication of USQ Graduation Documents Request form
Use this form if you are submitting original (not copies of) print official graduation documents for authentication by USQ officials.
