System Access Forms

Security Forms are used by the Security Officer to determine what access you will require. Each form will be reviewed on an individual basis. Please ensure the form is correctly completed to avoid delays as incorrect forms will be returned to the staff member.

Agents acting on behalf of the university who are off campus and require view only access via the web to the Student Management System must complete this form and fax it to USQ International Office for approval.

Course Class Access Control Form for Academic staff. This form is used to give access to Gradebook, USQ Studydesk and EASE.
