Incident, injury or hazard reporting
UniHIRTS is the University's system to report and manage all safety related incidents and hazards. The system is now available for online incident and hazard reporting and for conducting follow-up actions from these reports. You will require your USQ username and password to use this system. Paper reports are no longer accepted but may be used to capture appropriate details before submitting the report online.
Incident reports: The University requires that any individual who is injured or involved in an incident at the University must inform their Supervisor as soon as possible. Individuals should familiarise themselves with the Incident or Injury Reporting Procedure before accessing UniHIRTS online and submitting an incident report.
Hazard reports: If an individual sees any item of plant, any procedure or any incident which has caused or has the potential to cause injury to people or damage to property, and it cannot be immediately rectified, they must inform their Supervisor as soon as possible. Individuals should familiarise themselves with the Hazard Reporting Procedure before accessing UniHIRTS online and submitting a hazard report
UniHIRTS Version 2 upgrade: UniHIRTS now has the capability to automatically escalate reports that have not been actioned by a supervisor within seven business days. After a report is submitted, the supervisor is automatically sent an email with a link to the report so he/she can add their supervisor comments. If the supervisor does not add their comments within seven business days, the report will automatically escalate to the next level supervisor and the system will send an email and link to that supervisor. The system will continue to escalate every seven business days until it is either addressed or escalates through the DVCs to the Vice-Chancellor.
NOTE: to avoid unnecessary escalation, if your normal supervisor will be away from
work for seven days when you submit the original report, you should nominate an
alternative supervisor, which can be done when you submit the report online.
